The information you give will help decide if your child needs an Education, Health and Care (EHC) needs assessment.
It may be shared with professionals (like teachers, health workers, or social care) to help with this process.
Sometimes, information can be shared without your permission — for example:
→ If a child or adult is at risk of harm.
→ To help stop or investigate a serious crime.

What you need to do
Select who you are from the list:
- I am the parent/carer – if you are the child’s parent or legal guardian.
- I am the young person – if the young person is aged 16 or over and making the request themselves.
- I represent an education setting – if you work at a school, college, or nursery.
- I am an education professional – if you work in education but not directly for a school (e.g. educational psychologist).
- I am a health professional – if you work in health services (e.g. GP, paediatrician).
- I am a social care professional – if you work in social services.
- Other – if none of the above apply.
